Fortis Support provides fully funded, high-quality in-home nursing and personal care for Australian veterans and widows across Australia. Our goal is to help you maintain independence and comfort in your own home.
Are You Eligible?
To access the DVA Community Nursing Program at no cost, you generally need a referral from your GP or Hospital and:
Veteran Gold Card
Eligible for all clinically assessed nursing and personal care needs.
Veteran White Card
Eligible for conditions accepted as war-related.
Our Services

Clinical Care
Wound management, medication administration, and diabetes support.
Personal Care
Assistance with showering, dressing, and hygiene.
Complex Support
Palliative care, chronic disease management, and catheter care.

Falls prevention and exercise assistance.

Get a Referral
Ask your GP or Hospital Discharge Planner for a referral to Fortis Support.
We Visit You
We arrange a home visit to assess your health and home environment.
